Recovery Practitioner

Full description

We are looking for a Recovery Practitioner for Hull regarding a block booking.

Recovery Practitioner Role:

• Experience of supporting patients with substance misuse and alcohol support needs
• Experience of working within prison healthcare and risk management processes including ACCTs
• Responsible for case management and able to prioritise changing and complex needs of caseload
• Through care support from reception into prison to transfer/ release into the community and ensure continuity of care pathways
• Assessment, planning and review using the Drug and Alcohol Outcomes
• Deliver 1:1 and group interventions to support patient’s recovery journey
• Report writing
• Partnership working with key professionals involved in the patient’s life
• Familiar with System One
